Register with the Practice
We are delighted to welcome new NHS
patients to our Practice. Our
catchment area covers residents of BS8,
BS9, BS6, BS1 and parts of BS7.
Please feel free to call in and pick up
a registration pack from Reception.
To be fully registered you need to
provide us with:
- Photo identification e.g.
driving licence, passport
- National Health number –
available from your previous GP
- Proof of address e.g. utility
bill, bank statement, tenancy
agreement
It will also save time if you can
download, fill in and bring the
following forms:
